Tender Description

Family Focus is the offer from Cheshire East Council in respect of the national Troubled Families programme. This is to provide Outcome 5 of the Public Health target and support the Health and Wellbeing Strategy. The Family Focus Health Trainer provider(s) will:n1. Establish a Health Trainer Working Groupn2. Implementation of the Health Trainer pathwayn3. Identification and implementation of the referral criteria and protocol, including exclusions (A family will need to consent to being referred into the programme otherwise this will be considered an exclusion. If the Health Trainer Provider(s) feels that the referred individual does not have the motivation to change due their lifestyle or a particular inhibitor in their circumstances or that they present a risk to themselves or others this can be considered an exclusion and would need to be referred back to the key worker with the reasoning)n4. Design and implement referral documents, health trainer documents, and promotional materialsn5. Develop a monitoring and evaluation processes to monitor progress of individuals
